Serum and Blister Fluid Pharmacokinetics of Amikacin in Severe Burn Patients

چکیده
Objective: To characterize amikacin pharmacokinetics in serum and in blister fluid of severe burn patients to guide optimal treatment timing. Methods: Patients (N = 32) were divided into four groups based on amikacin administration timing and groups received drug minutes to hours after injury. In Groups A, B, C, and D, amikacin (400 mg, IV) was administered 3 4, 10, 20 and 30 h post burn injury, respectively (N = 8 for all groups). Next blister fluid and venous blood samples from 9 patients were obtained at 0, 0.25, 0.5, 1, 2, 3, 4, 5, 6, and 7 h after drug infusion. Amikacin concentrations were measured with a fluorescent polarization immunoassay and pharmacokinetics was deduced using DAS3.2.5. Statistical analyses performed with SPSS13.0. Results: Compared with normal values, t1/2z of amikacin from burn patients was shortened in serum but amikacin halflives in blister fluid was significantly greater than serum half-life values (p < 0.05). Groups A and B had greater pharmacokinetic values at each time point, and Group D did not achieve antibacterial concentrations of amikacin. Conclusion: Early amikacin administration in severe burn patients offers greater concentrations of drug in serum and blister fluids.

Opsonic activity of blister fluid from burn patients.
The combination of skin loss and immune depression after thermal injury predisposes burn patients to an increased risk of infection. Since the commonest site of infection in the burn patient is the burn wound itself, we elected to study the opsonic activity of locally produced blister fluid, from 18 thermally injured patients, for the two most common organisms colonizing the burn wound (Pseudom...

Amikacin clearance can be increased in burn injury, which is often complicated by renal insufficiency. Little is known about the impact of renal replacement therapies, such as continuous venovenous hemofiltration (CVVH), on amikacin pharmacokinetics. We retrospectively examined the clinical pharmacokinetics, bacteriology, and clinical outcomes of 60 burn patients given 15 mg/kg of body weight o...

The transport of cefmenoxime,a cephalosporin antibiotic, into three kinds of exudates (exudate from excoriated skin, exudate from burn wounds, and burn blister fluid)from blood was studied on 37 burn patients after they were given an intravenous bolus of 50mg cefmenoxime per kg body weight.Concentrations of cefmenoxime in serum and in the above exudates or fluid were determined by bioassay.Concent...

The optimal clinical care of burn blisters has not been determined. The effects of burn blister fluid and control serum on epidermal cell proliferation and differentiation were determined. Both burn blister fluid and serum decreased the cell responses necessary for healing of the burn wound by approximately 40%. The degree of suppression varied from 81% to 28% dependency on the specific burn bl...
متن کاملEffect of immunomodulator pyrimethamine and cimetidine on immunosuppression induced by burn blister fluid.
Despite recent advances in burn wound management, sepsis remains the main cause of death in patients resuscitated after major thermal injury. Increased susceptibility to infections has been related to severe suppression of the immune system. The aim of this study was to induce immune suppression with blister fluid injection, and to modulate immune response by use of cimetidine and pyrimethamine ...
